Record Requests

Please email all requests for records to:  admin@expansioncounselingservices.net 

Records request should include the following: 

  -Mailing address/email and/or fax number records are being sent to  

-Purpose of request

 -Signed authorization/consent to send records (if applicable; consent can be found on forms page) 

-$50 Deposit (administrative fee) for initial request up to 50 pages; an additional $1.00 for every page over 50; flat rate of $150.00 for an entire file
