Fees

*All cancellations must be made 24 hours prior to appointment to avoid a $25 fee   

FMLA, LOA or disability forms filled out at a flat rate of $150 per form and $75 flat fee for any additional forms (Please note that we are not taking any FMLA/LOA  referrals until further notice as of 8/16/21)   


Substance Abuse assessments fee of $150
